Patient COVID-19 Screening & Consent Form
When arriving to our office:
- Stay in your car
- If you come to your appointment with another individual only the patient will be allowed to enter (unless a child/elderly needing assistance)
- We will come to you and escort you into the office after we take your your temperature
- You will not be waiting
- Sanitary steps will follow (mask, handwash)
